What is FOXO4-DRI?
FOXO4-DRI is an engineered interfering peptide built to do one specific job: selectively destroy senescent cells. Senescent cells are aged, dysfunctional cells that stop dividing but do not die, and their buildup is linked to age-related decline. FOXO4-DRI is a senolytic, and it drew a lot of attention from a 2017 paper. It is also one of the least-developed compounds here: the evidence is entirely preclinical, and there is no human data at all.
How FOXO4-DRI works
Senescent cells survive partly because the protein FOXO4 holds p53 (a key cell-death regulator) in check. FOXO4-DRI is designed to break that FOXO4-p53 interaction, which releases p53 and pushes the senescent cell into apoptosis (programmed death), while largely sparing healthy cells. In the original studies it selectively killed senescent human fibroblasts with roughly a twelve-fold selectivity over normal cells.
What the research shows
The foundational work, published in Cell in 2017 by Peter de Keizer's group, showed that FOXO4-DRI selectively cleared senescent cells and, in aged and progeroid mice, improved markers like fitness, fur density, and kidney function. Follow-up studies have explored senescent-cell clearance in other cell types. Every bit of this is in mice and cell culture. There are no human trials, so the anti-aging promise is a compelling hypothesis at the earliest stage of testing.

There is no human dosing. All protocols come from mouse studies. Because nothing has been tested in people, any human use is genuinely experimental and unsupported by clinical evidence.

Side effects and safety
There is no human safety data whatsoever. Selectively killing cells is a powerful mechanism, and moving it from mice to humans safely is exactly the unsolved problem. This should be understood as a frontier research compound, not something with any real-world track record.
