SS-31, also known as elamipretide, is a mitochondria-targeted tetrapeptide and the most clinically advanced mitochondrial peptide by a wide margin. After two decades of research it reached a real milestone: in September 2025 the FDA granted accelerated approval to elamipretide, under the brand name Forzinity, for Barth syndrome, a rare inherited mitochondrial disorder. That makes it the first mitochondria-targeted therapy ever approved.

How SS-31 (Elamipretide) works

SS-31 is engineered to embed itself in the inner mitochondrial membrane, where it binds and stabilizes cardiolipin. Cardiolipin is the phospholipid that organizes the electron transport chain, the machinery that makes cellular energy. By stabilizing it, SS-31 helps preserve mitochondrial structure and function and reduces the oxidative damage that comes from leaky energy production. The targeting is highly specific to the mitochondrial membrane.

What the research shows

This is the rare mitochondrial peptide with a genuine human clinical program: numerous phase 2 and phase 3 trials across conditions, with systematic analyses of both successes and failures. The results are genuinely mixed. Preclinical models showed benefit in many settings, but several human trials, including in heart failure and primary mitochondrial myopathy, did not meet their primary endpoints.
