PT-141 (Bremelanotide)

What is PT-141 (bremelanotide)?

PT-141, or bremelanotide, is a melanocortin receptor agonist and one of the few compounds here with real FDA approval. It is sold as Vyleesi, approved in 2019 for acquired, generalized hypoactive sexual desire disorder (HSDD) in premenopausal women. What makes it interesting is the mechanism: it works on desire in the brain rather than on blood flow in the body, which is a genuinely different approach from the familiar erectile-dysfunction pills. It is also widely used off-label by men.

How PT-141 (bremelanotide) works

PT-141 activates melanocortin receptors in the brain, primarily MC4R (and MC3R), which sit on the central pathways involved in sexual arousal and desire. This is the key contrast with PDE5 inhibitors like sildenafil (Viagra): those work on the vascular system to enable an erection, while PT-141 acts on the nervous system to influence desire itself. That is why it can help with the "interest" side of things, not just the mechanical side, and why it is dosed ahead of time rather than continuously.

What the research shows

Bremelanotide earned approval through two randomized, placebo-controlled phase 3 trials (the RECONNECT program) in premenopausal women with HSDD, which showed statistically significant improvements in sexual desire and in the distress associated with low desire. That is real, regulator-grade human evidence. The honest boundary: the approved evidence is in premenopausal women. Its popular off-label use in men for erectile response is supported by earlier research and clinical experience but is not the population the pivotal trials studied.

Dosage and how it is used

The FDA-approved dose is 1.75 mg by subcutaneous injection, taken as needed at least 45 minutes before anticipated sexual activity. The label caps use at one dose per 24 hours and no more than eight doses per month. It is an on-demand product, not a daily one, which fits its brain-level, arousal-timing mechanism.

Side effects and safety

The most common side effect by far is nausea, reported by roughly 40% of users, usually mild to moderate and often easing after the first dose or two. Flushing, injection-site reactions, and headache are also common. Importantly, PT-141 causes a transient rise in blood pressure and a small drop in heart rate after dosing, so it is not appropriate for people with uncontrolled hypertension or known cardiovascular disease. It can also cause focal darkening of the skin or gums with repeated use, a melanocortin-pathway effect.
