What is Semax?

Semax is a synthetic peptide based on a short fragment of ACTH, the adrenocorticotropic hormone. Chemists in Russia stripped that fragment (positions 4 to 10) of its hormonal action and stabilized it, leaving a molecule that reaches the brain but does not drive the adrenal glands. It has been a registered prescription drug in Russia since the mid-1990s, given intranasally for stroke recovery and cognitive complaints. It is not approved in the US or EU, so outside Russia it is a research compound.

How Semax works

The headline mechanism is neurotrophic. In animal studies a single dose raises brain-derived neurotrophic factor (BDNF) and its receptor trkB in the hippocampus, the region tied to learning and memory. Semax also nudges dopamine and serotonin signaling and interacts with melanocortin receptors, which may explain its reported effects on attention and arousal. Crucially, because it is only a fragment of ACTH, it does not stimulate cortisol release the way the full hormone does, so you get the central effects without the stress-hormone spike.

What the research shows

Semax has been studied mainly in Russia, where trials looked at ischemic stroke recovery and chronic cerebrovascular problems. That gives it a longer clinical track record than most nootropic peptides. The honest caveat is that most of those trials are published in Russian, used sample sizes that would not clear a Western Phase 3, and have not been independently replicated by outside groups. The BDNF and trkB findings are solid but come from rats. So the mechanism is well described in animals and the human use is real, but the high-quality outcome evidence Western reviewers look for is thin.

Dosage and how it is used

There is no approved dose outside Russia. In the approved market it is dosed intranasally, and community protocols copy that route. Concentrations vary widely between products, and none of the self-use figures come from standardized dose-finding trials, so treat any number you see as convention rather than validated medicine.

Semax is generally reported as well tolerated, and decades of Russian clinical use have not surfaced major safety alarms at approved doses.
